A support service that provides supervised programming in a group setting for adults to assist them to achieve and maintain their maximum level of functioning, to prevent premature and inappropriate institutionalization, and to provide respite and information for caregivers.
May include: planned social, recreational and physical activities, meals, and transportation (if required), personal support/attendant care and minor health care service (e.g., monitoring medication).
Usually located in a hospital, senior’s centre or other community location, the service is provided for all or part of a day, or, in a few locations, during the evening. User fees may apply.
Frail seniors, persons in early/moderate stage of dementia, stroke recovery patients. Must be capable of one person transfer.

Cost of service: $16.00 full day. $10.00 half day.Hot meal and snacks are included, but transportation is not. If a subsidies is requested a financial assessment is done to determine an affordable fee.
Monday to Friday 9:00 a.m. to 3:30 p.m. (flexible)
Day program staff will help coordinate transportation to accommodate needs whenever possible. Main transportation provider for St. Clair O’Connor is Harmony Hall (416-752-8868) but available only for clients living in East Toronto. Clients from outside East Toronto area are welcomed if they can arrange alternative means of transportation.
Activities include crafts, music, gentle exercises, games, special celebrations, pet program, cooking/baking, walking group, monthly birthday party, spa program, current events, intergenerational program, and indoor/outdoor gardening.
