Initial assessments free, ongoing case management $99.00 per week
Initial Assessment with Client Care Manager. Weekly visits by an experiences Client Care Manager. Provide training and review of basic personal care techniques, ongoing supply of documentation, 24-hour on-call support fro questions, urgent matters. Aid in navigating the health care system, arrangements for medical equipment and supplies, communication with other health care professionals, advocacy. Immediate response for additional care.
