Crisis Support and Assistance
TransCare Community Support Services
Support and assistance for a senior in a crisis situation, including planning and coordinating other services. May develop into ongoing Case Management, with a follow-up plan to prevent further crises.
People who may require support in accessing services. Seniors and adults with disabilities who do not have any formal family support and/or caregivers needing assistance in navigating health and social services for their aging parents and/or loved ones.

Crisis Intervention and Support: Services provide in-home and in-office assessments to identify needs, facilitate access to information and community resources, and develop short and long term service plans. Monitoring and follow-up service is also available.
