A day program intended to assess and/or treat individuals through attendance at a specialized hospital unit. Attendance ranges in time.
Transportation and meals may be provided. Fees may apply.
Wellness Path Program : Frail seniors 55 and over having difficulty functioning in their homes.
Falls Prevention Program: Adults 65 or older living in the community who have a fallen or have a fear of falling

Jewish, Principles of Judiasm guide daily operations while service is provided to clients of all backgrounds.
Wellness Path Program : Covered by OHIP.
Falls Prevention Program: No charge to patient other than a nominal fee for the manual that participant keeps
Wellness Path Program : an outpatient rehabilitation program for elderly people living in the community who are experiencing challenges at home. We use a team approach to help address needs. The ultimate goal is to is to help clients live independently in the community. Note: Clients attending the program must need the intervention of two or more disciplines including: physician, nursing, occupational therapy, speech language pathology, dietary, physiotherapy, recreational therapy and social work. At discharge clients are usually linked to a community program.
Required: Physician completed and signed medical referral highlighting 2 or more disciplines and concerns with functioning i.e. falls, difficulty bathing, swallowing and/or caregiver stress to be considered for the program. Clients are seen for an interdisciplinary assessment and, if accepted, begin the program.
Required attendance: Must attend 2 times per week for 3 hours each day for a total of 12 weeks. Monday/Wednesday 1:00 to 4:00 p.m. or Tuesday/Thursday 9:00 a.m. to12:00 noon

Falls Prevention Program:
Interprofessional risk management, education and exercise program for older adults living in the Community who have experienced one or more falls or near misses or have a fear of falling. The goal is to minimize the risk of future falls.
Monday and Wednesday mornings from 9:00 a.m. -12:00 noon
