Services geared to individuals diagnosed with dementia before the age of 65 or their families.
Adults under 65 experiencing memory loss or cognitive impairment are eligible for assessment through our clinic, with details of the service below. We accept referrals from patient family members or significant others, through our website, with the consent of the patient, of course.

The clinic has both OHIP and non-OHIP insured services. There is no special additional fee for this service. It is included in our initial non-OHIP fee of $950, which includes the comprehensive assessment detailed above. The fee to the client for the non-OHIP insured services is $850, payable at the feedback session. Clients with 3rd party insurance will be eligible for reimbursements.
Services available at clinic or in home, including assessment, counselling and education.
The primary evaluation by the Geriatrician and the Psychologist occurs at the clinic.
However, a home visit by an Occupational Therapist is an integral part of our assessment.
Comprehensive
Geriatric Assessment, including screening for Cognitive and Affective disorders;
Formal Cognitive Assessment by a Ph.D. Psychologist;
Functional and safety assessment, as part of a home visit, by a registered Occupational Therapist;
Access to Social Work support as needed by client and support network;
Feedback session with Dr. David Tal, integrating multidisciplinary assessment.
Most referrals seen within 4 weeks, and completed within 8 weeks.
