Geri Health Home Care is guided by primary Home Care Model: Client is assigned to a designated PSW for consistency and continuity of care. All patients have care manager who facilitate care transition, do an initial, ongoing assessment and evaluation of care provided. Scheduled home visit, do health assessment and check BP/Blood sugar. Review health status with patient. Revised care plan as needed. A complementary service for our clients.
We communicate, coordinate and work collaboratively with client and their family to develop a plan and strategies. Design a specific dementia program that is engaging and stimulating for the client to promote resilience and maintain independence and quality of life.
