Outpatient Geriatric Clinic: Comprehensive geriatric assessment for persons with complex medical and/or cognitive decline is performed assessing for reversible causes of dementia and/or recommendations to assist patients and caregivers in managing overall health and cognitive decline/dementia in-home.
Required: Complete signed medical referral to the intake secretary is required highlighting concerns with functioning i.e. falls, difficulty bathing, swallowing, and/or caregiver stress
