A service that assesses the potential physical dangers faced by people with memory and cognitive deficits living alone or with family. Solutions to these potential dangers are proposed and/or arranged.
Population Served:
People of all ages recovering from a hospital stay, surgery or illness, or require support to remain at home.

Service Limitation:
What you need to receive LHIN services:
– a valid Ontario Health Card
– have a need for the service
– unable to go out of home to receive health care service
– needs that cannot be met on an outpatient basis
