This service provides support to seniors who have been discharged from hospital care and are returning to their personal residence. May include: Transportation to home from hospital; Trained escort (PSW) from hospital to home; Support to senior and caregiver to get settled at home; Fees may be charged for this service.

Service Limitation:
A registered Nurse manager will provide an assessment to meet the needs of the client. We have a quality assurance program in place to regularly check that the care you receive meets our high standards.
