This service provides support to seniors who have been discharged from hospital care and are returning to their personal residence. May include: Transportation to home from hospital; Trained escort (PSW) from hospital to home; Support to senior and caregiver to get settled at home; Fees may be charged for this service.

Home at Last program supports seniors to settle at home when they are discharged from William Osler Health Centre, including Etobicoke General Hospital, Brampton Civic Hospital. Services include: transportation home from hospital, accompaniment by PSW from hospital room to home, and required support to help patient and caregiver, as required, to get settled at home, up to 9 pm.
