This service provides support to seniors who have been discharged from hospital care and are returning to their personal residence.
May include: Transportation to home from hospital; Trained escort (PSW) from hospital to home; Support to senior and caregiver to get settled at home; Fees may be charged for this service.

Population Served:

For persons with dementia accompanied by a caregiver.

Service Limitation:

Home Instead Senior Care’s Caregiver’s can provide the necessary assistance to ensure a smooth transition from hospital. Services may include transportation, light housekeeping, grocery shopping, appointment management, Personal Care, and assistance with other tasks that may be a challenge in your first days back home.
