Mosaic is internationally recognised provider of person centered community integrated home care. Our model addresses the wider social and emotional needs of the person and their families both in their homes and in their communities.
Our community resource centres and our social outreach programs, delivered through our community resource centres and through many of our community partners, are currently affeted by the important social distancing response to the NOVEL-COVID-19 virus. The timeline on the resumption of these community services is uncertain.
Our present focus is on the safe and effective delivery of important care within protocols that minimise the risks of viral transmission to the people we look after, their families, our care workers and our community.
From the initial assessment to creating your care plan, from caregiver recruitment, selection and personal caregiver introduction to home care checks our experienced clients services team and caregivers, personalize, oversee and adjust care as needed. We are here for you and your family 24/7. Care by the hour (minimum of 3 hours of care), overnight care, 24-7 care.
Our services are uniquely focused on supporting and enhancing the social and emotional, as well as physical needs of those we care for. We commit to knowing the person, to meaningful conversation, to interaction with interests, activities and where possible the wider community.
Through person centered care and our community outreach we also commit to developing age friendly communities and healthy ageing across the GTA.
Services may be arranged for short-term or longer-term and can be provided in the home, in the hospital, in a retirement residence, or in a long-term-care facility. In order to meet various levels of needs, we offer a range of categories for care and support; these include:
Home Care: Personal Support, Homemaking, Specialised Care, Central Oversight by our client services team, Elder Caring Plus (Social work, client services, integration and communication, client & family working as an integrated team) within our home care model.
Medical: Nursing, Palliative Care & Pain Management
Social, Emotional, Lifestyle
The Meaning of Me – our ongoing conversation framework, hobbies, interests, discussion, goals, social networks, community. Mosaic’s person centered care model of care.
Person Centered Certification for our staff
Lifestyle Companions – outings, conversation, cooking, shopping, food preparation. No medical care is provided under this level of care.

Prices may change during COVID-19 Pandemic. As we provide pandemic pay to our caregivers and advanced contact tracing protocols, personal protective equipment and oversight with regards to keeping our clients, families and personal support workers safe in our communities.
Prices range from 28.25 per hour to $32.00 per hour

Mosaic Home Care provides an Elder Care Plus service - Personalised Advanced Care Planning and Oversight options.
At Mosaic our client services team posesses social work, nursing, palliative care, end of life, grief counselling and geriatric care expertise. All our clients/members benefit from this expertise as part of our primary home care services oversight.
However, ther are times when the needs of our clients/members and their families extend beyond the realm of primary home care services. Knowing what to look for, what your options are, when to bring in outside help and how to manage difficult transitions can be of utmost importance. Our experienced team can help you seamlessley plan for and manage these transitions.
