Service Limitation:

A referral from a family doctor or specialist is required. Self-referrals are subject to a screening process to determine suitability for clinic services. Arrangements can also be made by clinic staff to provide service in various languages, as needed. Please call for a central intake and referral form.
Includes a comprehensive report that incorporates recommendations from various disciplines like neurology, psychiatry, geriatric medicine, psychology, nursing, and occupational therapy.
