Evaluates the individual from the perspective of more than one health profession. Nurses, physicians, nutritionists, occupational therapists and physiotherapists are frequently involved.
Seniors, individuals recovering from surgeries, injuries, Acquired Brain Injury, individuals with special needs, ALS, dementia, alzheimer’s. We provide accompanied transportation.
Multi-disciplinary Assessment
Nursing Services at Home – Minimum of hours per visit: 3, Cost per hour: $36.00
Palliative Care/Hospice – Minimum of hours per visit (if service is provided at home): 3, Cost per hour: $24.95
Live-in Caregiver – approx. $4200.00
Personal Care Services at Home (Respite Care at Home) – Minimum of hours per visit (if service is provided at home): 3 hours, Cost per hour: $24.95
Home Safety Assessment and Counselling for Persons with Dementia and Caregivers – Home Safety Assessment $125
Medical Response Alarms – Depends on unit…plus $25/month monitoring fee. Web based tracking. Unit is a GPS locator as well as a one button voice communication device.
Dementia Specific Education and Training – Our own Pathways to Memory program, $24.95/hr
Homemaking – Minimum of hours per visit: 3, Cost per hour: $22.95
Transportation Services – Cost: $24.95 PLUS .65/KILOMETER (if using caregivers vehicle) , Accompanied transportation. Our caregivers are insured to drive the clients vehicle to to use their own vehicle. Or, will accompany the person on public transportation or any other arranged transportation.
Pathways to Memory is Homewatch CareGivers unique approach to dementia care. This specialized form
of care features one-on-one interaction and attention and provides social interaction and companionship to
the person living with dementia. Flexible home care services plans that cover a few hours a day or caregiver shifts around the clock provide relief for family caregivers, including bathing, dressing and toileting, specialized personal care related to immobility, picking up prescriptions, personal shopping, and support through the end of life and grief process.
