Evaluates the individual from the perspective of more than one health profession. Nurses, physicians, nutritionists, occupational therapists and physiotherapists are frequently involved.
Usually 65 or older. Clients usually experiencing complex web of medical, functional and social problem that interfere with wellbeing and ability to function.
(Geriatric Clinic): Steeles Avenue to St. Clair Ave, Avenue Road (W side) to Dufferin Street
(Memory Clinic) Open catchment area.

Covered by OHIP.
GERIATRIC CLINIC After the comprehensive nursing assessment is complete, a referral may be made to any one of the following: physiotherapy, occupational therapy, social work and all patients are seen by the geriatrician, either in clinic or in the home.
Required: A complete signed medical referral to the intake secretary is required highlighting concerns with functioning i.e. falls, difficulty bathing, swallowing, and/or caregiver stress.
