Toronto including Toronto Islands, former City of York, Borough of East York, southern part of Etobicoke



Cost:

No cost to eligible clients.




Service Limitation:

An assessment is necessary for determining whether clients are eligible for CCAC services.
What you need to receive CCAC services:
– a valid Ontario Health Card
– live in the catchment area
– have a need for the service
– must be unable to go out of home to receive health care service.
