GERIATRIC CLINIC: Psychiatrist and allied health professionals provide in-home assessment within catchment area or office visits if outside catchment area and no alternative service available. Consultation with geriatric medicine and other medical specialties may be requested as needed.
Required: Referral to team from any source, provided that family physician is informed is in agreement with referral and will provide information.

MOOD AND RELATED DISORDERS CLINIC: A comprehensive mood assessment is carried out by a psychiatrist and other team members through interviews and pencil-and-paper questionnaire
Required: Referral to team from any source, provided that family physician is informed is in agreement with referral and will provide information.
