Service Description: Providing home health care services * Nurse managers do a free in-home assessment to discuss each client and their family’s needs and offer reasonable options to consider * can provide services with very short notice at home, at the hospital, in rehabilitation facilities, LTC facilities and in retirement homes * staff liaisons and collaborates with hospitals and government service providers to help with successful and seamless discharges home from hospital or rehabilitation facilities
