A support service that provides escorted or unescorted transportation to medical appointments, shopping and to various social activities and programs. Services may vary between agencies. Fees may apply.
Population Served:
Persons 55 years and over and those with physical disabilities, memory impairment due to Alzheimer’s disease or related disorder, or who are frail or isolated.
$7.00 round trip to Adult Day Program. Transportation service cost to doctor’s appointments, hospital and grocery shopping is based on mileage. Client is responsible to pay parking fees.
Service Limitation:
Book in advance (at least one week before your appointment). A reminder telephone call is also provided the day before the scheduled appointment.
