A support service that provides escorted or unescorted transportation to medical appointments, shopping and to various social activities and programs. Services may vary between agencies. Fees may apply.

Population Served:

Persons with dementia travelling alone or accompanied by a caregiver.

Eligibility:

Users must be members of and registered with our Transportation Dept.

Users must live within our catchment area

Users are unable to use public transit because of physical or cognitive limitation

Users are not eligible for Wheeltrans service

Our vehicles are not wheelchair accessible. Users must be able to access the vehicle independently or with minimum assistance

Users must be members of NYSCs Active Living Centre

Bathurst to Victoria Park, Steeles to Hwy 401

Cost:

Rides within NYSC boundaries: Bathurst St. to Victoria Park Ave. and Steeles Ave. to Hwy 401 – $5.50 each way

Exceptions are made for the following destinations:

Branson Hospital – $6.00 each way

Valleyview Residence – $6.00 each way

240 Duncan Mills Road – $6.00 each way

Baycrest Centre for Geriatric Care – $7.00 each way

Rides to 7131 Bathurst St. – $7.00 each way

Sunnybrook Health Centre, Toronto Rehabilitation Centre, CNIB – $8.00 each way

Service Limitation:

Services Include: Rides to and/or from medical appointments, NYSC day program, NYSC Active Living Centre, and a shopping program to Centrepoint Mall, every Wednesday and Friday

Hours of Operation: Rides are provided from 8:30 a.m. to 4:30 p.m.
