Initial assessment sessions $150 (1.5 hours in length) and follow up visits are $100 (1 hour in length)
CASE MANAGEMENT: assisting clients and their caregivers to navigate Ontario’s complex health care system, helping families struggling to come up with a care plan, helping families decide if placement in a long term care facility is really necessary vs. aging at home
HOSPITAL ADVOCACY: help when your loved one is in hospital, we can attend discharge planning meetings, set up needed services at home for your loved one and conduct follow up home visit assessments to ensure your loved one is managing post discharge from hospital
