Conducting an assessment, planning and coordinating services to meet a senior’s needs, goals and expected outcomes. Service may be provided at a senior’s home or other location.
May include: advocacy to assist seniors or caregivers in accessing needed services, to help them avoid “falling between the cracks” during service delivery.

Population Served:

Persons with dementia + caregivers + referrals from clients/caregivers/friends/community agencies and health-care professionals.

Cost:

Initial assessment sessions $150 (1.5 hours in length) and follow up visits are $100 (1 hour in length)

Service Limitation:

• CASE MANAGEMENT: assisting clients and their caregivers to navigate Ontario’s complex health care system, helping families struggling to come up with a care plan, helping families decide if placement in a long term care facility is really necessary vs. aging at home

• HOSPITAL ADVOCACY: help when your loved one is in hospital, we can attend discharge planning meetings, set up needed services at home for your loved one and conduct follow up home visit assessments to ensure your loved one is managing post discharge from hospital
