Client Intervention and Assistance

This program offers crisis intervention, client service assistance and information for seniors, people with dementia and/or caregivers living in the community. Please note that this is not an emergency response service. Client service assistance includes problem identification, referral and services coordination and discharge program.
May include: Supportive counselling to seniors and their caregivers; Information and referral to community resources for identified needs or issues; Coordination of services; In-home visits; Practical assistance with regard to housing, finances, medical needs, income programs and accessing community services; Practical assistance such as filling out forms and applications (income tax, old age security, income supplement); Crisis intervention; Advocacy and assistance to people who are at risk of being unable to continue living independently in the community.

Client intervention and assistance
Jewish, Principles of Judiasm guide daily operations while service is provided to clients of all backgrounds.
No extra charge – part of the Day Program cost.
Service consists of an interdisciplinary team in contact with family, family physician and other professionals. Our team includes a social worker, registered nurse and program coordinator)
