Support and assistance for a senior in a crisis situation, including planning and coordinating other services. May develop into ongoing Case Management, with a follow-up plan to prevent further crises.
Population Served:
Seniors, who are physically frail, are convalescing, have physical and/ or mental health challenges and/ or have Alzheimers disease and/or dementia, and their caregivers.
Catchment Area:
North to Lawrence Avenue West, South to St. Clair Avenue West, East to Spadina Road and West to the Humber River.
