Crisis Support and Assistance

Support and assistance for a senior in a crisis situation, including planning and coordinating other services. May develop into ongoing Case Management, with a follow-up plan to prevent further crises.
This service is available for participants of the Adult Day Program
East of Dufferin St/Vaughan Rd; north of St Clair Ave; west of Oriole Pkwy; north of Eglinton; west of Yonge St; north of York Mills Rd; west of Leslie St; South of Centre St from Dufferin St to Yonge St; South of Steeles Ave from Yonge St to Leslie St
Crisis support and assistance
Jewish, Principles of Judiasm guide daily operations while service is provided to clients of all backgrounds.
No extra cost. Service is provided as part of the Adult Day Program. No charge in the Community Outreach Team
Multidisciplinary team provides support and counselling as part of the Alzheimers Day Program. Care Outreach Team will make recommendations in crisis situations in the best interest of the client/caregiver with family involvement. Placement may have to be arranged in a crisis situation.
Service provided by: Community Day Centre for Seniors: Parkland, Oceanside and/or Samuel Lunenfeld Mountainview Clubs and the Community Outreach Team and the Saul and Rae Saltzman Day Treatment Centre.
