An oral examination by a psychiatrist or other physician, designed to elicit or identify psychiatric dysfunction, such as anxiety or depression (depression may be defined as a reversible dementia). The assessment is comprised of a full history and mental status examination.

Population Served:

Individuals 60 years of age or older who: are experiencing a mental health issue; display significant changes in mood and behaviour; have difficulty managing safely due to changes in their mental function; are medically stable; reside in the community (including a retirement residence).

Also available to care givers experiencing stress while caring for a relative and who would like information about local community services and long-term care facilities.

Service Limitation:

An assessment is conducted in the client’s home by a mental health professional such as a social worker, nurse or occupational therapist with special training in geriatric mental health. They will help the client review the present challenges and work with the client to problem solve. Issues may be stress, depression, anxiousness, confusion and dementia, difficulties arising from memory loss.
The worker is trained to complete memory testing, if necessary and can review medications and other health factors that may be impacting client’s mental health. Family members are welcome to attend and be active participants in this process.
Consultation with psychiatrist to determine most appropriate intervention plan.
Clients and families are provided practical advice to help individuals and families manage difficult behaviour, stress, depression, confusion and Alzheimer’s disease or related dementia.
The mental health worker will liaise with family physician as needed. Written recommendations for treatment may be forwarded to the family physician.
